Documents required in case of death claims

  • Claim forms (Death Claim Form & Attending Physician's Report)
  • Death certificate issued by Union Council or NADRA
  • Hospital death certificate
  • CNIC of claimant and deceased
  • Original policy documents
  • All medical records

Further, a copy of Policy FIR, Post Mortem report, newspaper cutting will be required in case of 'accidental' death. Complete set of medical records of assured may also be required in some cases.

  • Disability claim form
  • Life care claim form
  • Proof of disability / illness
  • All medical records

In addition to the basic documents, further requirements or documents may be called at any point of claim assessment depending on nature of claim.
